There are a few things that can cause this, as in the state of tune the engine is in. I know you say it runs fine after the initial start but when was it last tuned-up?
If your truck were here at the shop for me to check and we knew that the tune-up parts were ok. I would have a fuel pressure gauge connected to the system for its first start. That’s to make sure that the fuel pump was capable of producing the proper fuel pressure.
The spec on the running pressure is 60 to 66 psi and with this style of central fuel injection the engine won’t even think of firing if the pressure is below 60 psi.
I’m not saying this is the problem but it’s the staring point as in where to look.
Dan.

Dan, thanks for the prompt feedback. Let me take you back to the beginning. It started to get a lil rough to start in the mornings only and i figured it was time for a tune up. Replaced plugs, wires, cap, rotor, fuelfilter changed oil and installed fuel injector cleaner. The morning starts remained stubborn, but she ran alot better. And slowly over the past 2-3 months its been getting harder and harder to start, now it will turn over 5-8 times then i shut her down, recrank and it starts up, and will start up everytime on the first crank throughout the day. It seems that when it sits for any extended period of time is when the hard starting occurs. Ive seen where you talked about a fuel pump relay in previous posts, whats the chance thats the culprit? Oh and when i turn ignition to acc. i definately hear the fuel pump prime for its 2-3 seconds. ????????????? Thanks for any advice.
Bubba Out.
