1995 saturn sl - car over heating

I have a 1995 saturn sl.( 120,000) it starts to overheat with out the ac on and when at an idle. with the ac on it ok. it does have coolant. the fan turns on with ac but seems to turn off with out it. I checked the relays and fuses. all ok.

It's normal for it to get hotter at idle with the A/C off. the fans are programmed to come on at a much higher temp. The temp does not get into the red zone, yet the coolant still comes out of the overflow cap on the right side of the engine. The fan did not work and I changed out the fan and it is working, I have checked the relays and fuses, yet it still gets hot and overflows. I have read about the engine coolant temp sensor and thermostat, would this be the next step?

the cap isn't holding, the fluid is coming out the cap

It's supposed to hold a specific pressure before it releases.
