 Throwout Bearing? please help
88 dakota 3.9 2wd 5 speed hydraulic chutch

Last year i put in a new dynapac clutch kit with new throwout bearing.
A few days ago there were come clicking when i put load on the motor on the highway and it went away when i depressed the clutch. I checked the oil level in the trans and it was full. Today the noise in unbearable and i know something is messed.

The first thing i checked was the output shaft bearing and u joints, then the slave cylinder which i think was the cause of the problem.

The rubber on the slave cylinder rod had a indentation in the middle like it wasnt sitting in the groove on the throwout bearing collar, but rather on the edge which is 1/2" closer to the tip of the cylinder rod and off center. Maybe the extra pressure off center caused the bearing to fail prematurely. The slave cylinder rod also came out of the slave cylinder when i took it out of the bellhousing is this normal?

Heres when it makes the noise:
Sitting at idle (the slave cylinder rod is sitting out of the groove and i think is pushing on the collar causing the bearing to turn and chatter)
Driving in gear

It wont make the noise when the clutch pedal is pressed down driving or not, so that would eliminate the transmission bearings and gears because they still are turning when i am moving correct?

Could the slave cylinder could of been worn out and pushed the throwout bearing off center to cause wear, and now chatter?

When clutch is depressed the front shaft of trans is not turning at engine speed. Basically when clutch is drpressed the vehicle is ignoring the trans alltogether except for input from driveshaft which is vehicle speed directly. At first blush I suspect the trans front shaft bearing,
