89 gmc k1500 won't shift

I have a 1989 GMC K1500 4X4, 5.7 ltr, 4 speed with over drive. The other night I drove home and when I turned around, I had no reverse. I rolled the truck down, turned around using gravity, and only had 3rd or 4th. I took the shifter off thinking it might be the linkage and found the coupler (part that the shifter sits in) loose on the top shaft of the transmission. Now, when I can get it in neutral, it sounds like it's grinding. Can I just tighten this up? If so, does anyone know what tool I need. I had no problems before this and there was absolutely no indication that anything was wrong. Thanks for your help. Darrin

How many miles is on this vehicle? Do the shifter feel like its going in gear? Have you checked the fluid?

168K on truck, No, the shifter only goes into 3rd or 4th.

Have you checked the fluid level, and the grinding in neutral is a very bad sign , the gears could be damaged by now and you are looking at an overhaul /or used trans by inspecting the trans fluid for metal shavings would give a indication of how severe it is
