How is the shifter? Have you inspected the cables?

The shifter seems fine and I've followed the cables but can't get in there enough to go all the way to the transmission

? Just a thought and slight maybe for this - will it shift with key on but engine not running? That would be what can happen if hydraulic clutch fails to release clutch fully but not totally consistent with the trouble with 4th gear if up shifting,

OK - that rules that out. Descretesignals (DS) will be by for more. I don't have a procedure to access cables for this,

You should go to the transmission itself under the hood and look at the linkage there where it connects. Inspect that while someone moves the lever.
