Tranny making noise

2009 town and country-3.8 engine 161,000 miles
When I’m shifting from p to d or r I can hear the tranny kinda kicking into gear same thing is happening when driving when it kicks into gear, thought it was because of bad motor mounts because there was movement, so I changed motor mounts, I can’t afford another tranny so I hoping to get some help on here what to do next
